The association helps community health centers improve population health and reduce health care expenditures by providing research, advocacy, education, training and technical assistance. A three-year grant underwrites the joint development and implementation of a standardized health risk-assessment tool that enables health centers to consider social and environmental influences, as well as clinical health indicators, in their approach to patient care.
