The mission of the Alameda County Public Health Department is to work in partnership with the community to ensure the optimal health and well being of all people through a dynamic and responsive process respecting the diversity of the community and challenging us to provide for present and future generations. This grant will support the Alameda County Public Health Department’s efforts to create a process and system for sharing and tracking data to ensure better coordinated case management and data collection on race and social determinants of health across 30 public health programs and human service providers.
